WASHINGTON (AP) ? While the rest of us have to wait until June, the justices of the Supreme Court will know the likely outcome of the historic health care case by the time they go home this weekend.

After months of anticipation, thousands of pages of briefs and more than six hours of arguments, the justices will vote on the fate of President Barack Obama's health care overhaul in under an hour Friday morning. They will meet in a wood-paneled conference room on the court's main floor. No one else will be present.

In the weeks after this meeting, individual votes can change. Even who wins can change, as the justices read each other's draft opinions and dissents.

But Friday's vote, which each justice probably will record and many will keep for posterity, will be followed soon after by the assignment of a single justice to write a majority opinion, or in a case this complex, perhaps two or more justices to tackle different issues. That's where the hard work begins, with the clock ticking toward the end of the court's work in early summer.

The late William Rehnquist, who was chief justice for nearly 19 years, has written that the court's conference "is not a bull session in which off-the-cuff reactions are traded." Instead, he said, votes are cast, one by one in order of seniority.

The Friday conference also is not a debate, says Brian Fitzpatrick, a Vanderbilt University law professor who worked for Justice Antonin Scalia 10 years ago. There will be plenty of time for the back-and-forth in dueling opinions that could follow.

"There's not a whole lot of give and take at the conference. They say, 'This is how I'm going to vote' and give a few sentences," Fitzpatrick said.

It will be the first time the justices gather as a group to discuss the case. Even they do not always know what the others are thinking when they enter the conference room adjacent to Chief Justice John Roberts' office.

By custom, they shake hands. Then Roberts will take his seat at the head of a rectangular table. Scalia, the longest serving among them, will be at the other end. The other seven justices also sit according to seniority, the four most junior on one side across from the other three.

"They generally find out how the votes line up at the conference," said Orin Kerr, a George Washington University law professor who worked for Justice Anthony Kennedy nine years ago.

The uncertainty may be especially pronounced in this case, where the views of Roberts and Kennedy are likely to decide the outcome, Kerr said in an interview Thursday. "I don't think anyone knows. I'm not sure Justice Kennedy knows."

No one's vote counts more than the others', but because they speak in order of seniority, it will become clear fairly quickly what will become of the health care overhaul.

That's because Roberts speaks first, followed by Scalia, then Kennedy. If the three men hold a common view, the Obama health care overhaul probably is history. If they don't, it probably survives.

If Roberts is in the majority, he will assign the main opinion, and in a case of this importance, he may well write it himself, several former law clerks said. If Roberts is a dissenter, the senior justice in the majority assigns the opinion.

The court won't issue its ruling in a case until drafts of majority opinions and any dissents have circulated among the justices, changes have been suggested and either accepted or rejected.

"These justices aren't locked in. Minds have changed during the drafting process and minds have changed after opinions have been circulated," said Rick Garnett, associate dean and professor of law at Notre Dame Law School who worked for Rehnquist 15 years ago.

In one celebrated case decided in 1992, Rehnquist initially assigned Kennedy to write a majority opinion for five justices allowing prayers at public school graduations. In the end, Kennedy ended up writing the opinion for a different five-justice majority striking down the graduation prayers. According to several accounts, Kennedy simply changed his mind during the writing process.

No one will know precisely when decisions on particular cases will be coming, until perhaps Roberts ends a court session in late June by announcing the next meeting will be the last until October. Then it's a safe bet that whatever hasn't been decided will be on the last day. And decisions in the biggest cases very often aren't announced until that last day of the term.

Supreme Court opinions rarely find their way to the public before they are read in the marble courtroom, although the court inadvertently posted opinions and orders on its website about a half hour too soon in December.

The last apparent security breach occurred more than 30 years ago when Tim O'Brien, then a reporter for ABC News, informed viewers that the court planned to issue a particular opinion the following day. Chief Justice Warren Burger accused an employee in the printing shop of tipping O'Brien and had the employee transferred to a different job.

Sometimes, though, the justices themselves manage to let people know something big is coming.

On May 17, 1954, the attorney general, secretary of state and Nina Warren, wife of the chief justice, were in the courtroom when Earl Warren read the historic, unanimous opinion in Brown v. Board of Education outlawing school segregation.

Peter Nanula, former chief executive officer of Arnold Palmer Golf Management, started Concert Golf Partners in 2010. EL: What would you think if, more than 100 years from now, your best-known work would be ?The Battle Hymn of the Republic??

Howe: I would be pleased if my countrymen still would be willing to ?die to make men free.? You know, I sent that poem in 1861 to James Fields, editor of the Atlantic Monthly. I said, ?Fields! Do you want this?? He did. He took it.

EL: It was widely sung by 1883.

Howe:? Before it was published, I had thought there was nothing I could contribute to the war effort. I remember well the sad expression of Mr. Lincoln?s deep blue eyes. Much later I understood that I had made a contribution, a few words to strengthen the hearts of those who fought.

EL: Those words: ?Mine eyes have seen the glory of the coming of the Lord!? Where did they come from?

Howe: The day before I wrote that hymn, my husband and I were visiting in Washington and we rode out to attend a review of the Union troops at Baileys Crossroads. On our way back to town, we sang that new soldiers? song, ?John Brown?s Body.? We also saw the ambulances and the orderlies, the ghastly toll of war. I began writing at dawn.

EL: You also are known as a leader among women.

Howe: Women must rise. When I see their elaborate study and ingenuity in the pursuit of trifles, I feel no doubt of their capacity for the most herculean undertakings.

EL: You agreed with your late husband on important matters?in fact, he helped fund John Brown?s rebellion?but your husband tried to crush your aspirations. Why?

Howe: I still cannot fathom it. I never opposed him in anything because it would have been useless. I could not struggle with so fierce an opponent. But in 33 years of marriage, I never knew my husband to approve any act of mine. Everything was contemptible in his eyes because it was not his way of doing things.

EL: Yet you were not silenced.

Howe: My life has been a true expression of my faith. Why would I fear anyone? I have stuck to my resolution of writing what I thought, no matter whom it offended. God forgive me if I was wrong.

Radiotherapy for DCIS still protects against recurrence after 15 years

Vienna, Austria: Radiotherapy treatment (RT) after surgery for ductal carcinoma in situ (DCIS) [1] still has a major protective effect against recurrence more than 15 years later, according to the results of an international trial. Researchers found that the use of RT in addition to surgery could reduce the chances of a local recurrence (the cancer coming back in the same breast) by 50%. Results from the trial, which has one of the longest follow-ups of a large group of patients in the world to date, will be reported today (Thursday) to the 8th European Breast Cancer Conference (EBCC-8).

Dr. Mila Donker, a research physician from the Netherlands Cancer Institute, Antoni van Leeuwenhoek Hospital, Amsterdam, The Netherlands, worked with colleagues from thirteen other countries under the auspices of the European Organisation for the Research and Treatment of Cancer (EORTC), based in Brussels, Belgium. They analysed the 15-year follow-up of more than 1000 DCIS patients, 50% of whom had received RT after total surgical excision of a tumour of less than 5cm diameter and 50% who had not. When radiotherapy had not been given, almost one in three women had developed a local recurrence, while the 50% risk reduction in those patients who had had RT held true for both an in-situ and an invasive cancer recurrence, where the cancer had spread outside the duct.

The 15-year cumulative incidence for DCIS recurrence in the surgery group was 14.9%, as opposed to 7.5% in those patients in the combined group (surgery plus radiotherapy), and for an invasive recurrence the rates were 15.5% (surgery only) and 9.8% (combined). Although no survival difference was seen between the surgery alone and the surgery plus RT group, women who had an invasive recurrence had a significantly worse survival compared with those who had a DCIS recurrence or no recurrence at all.

"We found that the majority of the DCIS recurrences occurred within five years of treatment, and that RT seemed to have a continuous protective effect on DCIS recurrence in the long term," says Dr. Donker. "However, the protective effect of radiotherapy on the onset of invasive recurrence seemed to be observed mainly in the first five years after treatment, while the risk of developing a recurrence was more or less continuous over the years."

The number of DCIS cases has increased in the past few decades, and it now accounts for about 25% of all new breast cancer diagnoses. Until the introduction of routine screening it was often overlooked and not diagnosed until it had developed into invasive breast cancer. Because most breast cancers start in the cells that line the ducts and lobules of the breast, the diagnosis of DCIS is an important indication of a woman's risk of developing an invasive cancer.

Detecting an in situ lesion of the breast enables the patient to be treated at an early stage, but it also poses the problem of 'over-treatment' for a lesion that would not have developed into a life-threatening cancer. "That is why it is so important to be able to study the biological behaviour and the malignant potential of these in situ breast cancers in long follow-up trials. Our study provides convincing evidence that early treatment does work, and that it can help to avoid the more severe therapies that might be needed at a later stage," Dr. Donker will say.

"We now intend to analyse prognosis and treatment after recurrence in these patients. We now know that patients who do not receive RT have a higher risk of a local recurrence, but we will have a lot to learn about prognosis and treatment at a later stage. We intend to analyse the data to look at, for example, whether patients who received RT during the initial treatment have a higher risk of having to undergo a mastectomy after a local recurrence," Dr. Donker will say.

The researchers believe that their findings will allow them to better select patients who will benefit from particular treatments and avoid over and under treatment of patients with DCIS. "For example, in the future we will be able to identify patients who will not benefit from RT and spare them a treatment that might induce other tumours later in life. Or we could identify patients who need a more aggressive therapy than only local excision and RT patients with a high chance of recurrence might benefit from an initial mastectomy, for example. We are hoping that by learning more about the differences in outcome for these patients we will be able to make detailed recommendations for the best treatment to use if the cancer recurs, and hence be able to decrease yet further the local recurrence rate of DCIS patients in the future," Dr. Donker will conclude.

Professor David Cameron, from the University of Edinburgh (Edinburgh, UK) and chair of EBCC-8 said: "This is an important long-term follow-up study. DCIS is a much more common diagnosis now than when this trial was run, so that there are many patients whose care might be influenced by these data. These data also reiterate the importance of long-term follow-up in breast cancer, as patients can, sadly, develop a recurrence of their cancer many years after their initial treatment."

Vienna, Austria: Radiotherapy treatment (RT) after surgery for ductal carcinoma in situ (DCIS) [1] still has a major protective effect against recurrence more than 15 years later, according to the results of an international trial. Researchers found that the use of RT in addition to surgery could reduce the chances of a local recurrence (the cancer coming back in the same breast) by 50%. Results from the trial, which has one of the longest follow-ups of a large group of patients in the world to date, will be reported today (Thursday) to the 8th European Breast Cancer Conference (EBCC-8).

Dr. Mila Donker, a research physician from the Netherlands Cancer Institute, Antoni van Leeuwenhoek Hospital, Amsterdam, The Netherlands, worked with colleagues from thirteen other countries under the auspices of the European Organisation for the Research and Treatment of Cancer (EORTC), based in Brussels, Belgium. They analysed the 15-year follow-up of more than 1000 DCIS patients, 50% of whom had received RT after total surgical excision of a tumour of less than 5cm diameter and 50% who had not. When radiotherapy had not been given, almost one in three women had developed a local recurrence, while the 50% risk reduction in those patients who had had RT held true for both an in-situ and an invasive cancer recurrence, where the cancer had spread outside the duct.

The 15-year cumulative incidence for DCIS recurrence in the surgery group was 14.9%, as opposed to 7.5% in those patients in the combined group (surgery plus radiotherapy), and for an invasive recurrence the rates were 15.5% (surgery only) and 9.8% (combined). Although no survival difference was seen between the surgery alone and the surgery plus RT group, women who had an invasive recurrence had a significantly worse survival compared with those who had a DCIS recurrence or no recurrence at all.

"We found that the majority of the DCIS recurrences occurred within five years of treatment, and that RT seemed to have a continuous protective effect on DCIS recurrence in the long term," says Dr. Donker. "However, the protective effect of radiotherapy on the onset of invasive recurrence seemed to be observed mainly in the first five years after treatment, while the risk of developing a recurrence was more or less continuous over the years."

The number of DCIS cases has increased in the past few decades, and it now accounts for about 25% of all new breast cancer diagnoses. Until the introduction of routine screening it was often overlooked and not diagnosed until it had developed into invasive breast cancer. Because most breast cancers start in the cells that line the ducts and lobules of the breast, the diagnosis of DCIS is an important indication of a woman's risk of developing an invasive cancer.

Detecting an in situ lesion of the breast enables the patient to be treated at an early stage, but it also poses the problem of 'over-treatment' for a lesion that would not have developed into a life-threatening cancer. "That is why it is so important to be able to study the biological behaviour and the malignant potential of these in situ breast cancers in long follow-up trials. Our study provides convincing evidence that early treatment does work, and that it can help to avoid the more severe therapies that might be needed at a later stage," Dr. Donker will say.

"We now intend to analyse prognosis and treatment after recurrence in these patients. We now know that patients who do not receive RT have a higher risk of a local recurrence, but we will have a lot to learn about prognosis and treatment at a later stage. We intend to analyse the data to look at, for example, whether patients who received RT during the initial treatment have a higher risk of having to undergo a mastectomy after a local recurrence," Dr. Donker will say.

The researchers believe that their findings will allow them to better select patients who will benefit from particular treatments and avoid over and under treatment of patients with DCIS. "For example, in the future we will be able to identify patients who will not benefit from RT and spare them a treatment that might induce other tumours later in life. Or we could identify patients who need a more aggressive therapy than only local excision and RT patients with a high chance of recurrence might benefit from an initial mastectomy, for example. We are hoping that by learning more about the differences in outcome for these patients we will be able to make detailed recommendations for the best treatment to use if the cancer recurs, and hence be able to decrease yet further the local recurrence rate of DCIS patients in the future," Dr. Donker will conclude.

Professor David Cameron, from the University of Edinburgh (Edinburgh, UK) and chair of EBCC-8 said: "This is an important long-term follow-up study. DCIS is a much more common diagnosis now than when this trial was run, so that there are many patients whose care might be influenced by these data. These data also reiterate the importance of long-term follow-up in breast cancer, as patients can, sadly, develop a recurrence of their cancer many years after their initial treatment."

   Disagreeable is right in that ignorance of the law is not an excuse. You can't use the defense of not knowing your license was suspended in court, they will just laugh at you. You can plead guilty and pay the fine, or hire an attorney and attempt to fight the charges or at least get some type of better plea deal.
